There are two forms to fill out, and they do not work the same way. The McDevitt league form can be typed on a phone. The City of Philadelphia form cannot. It has to be printed and signed in pen.

You will need a printer at some point. Only for the City form. If you do not have one, call Earl at 215-681-5533 and he will get you a paper copy.

Registration closes Saturday, November 7, 2026. Players are placed on teams from the first two weeks of evaluations, so a late signup misses team assignment.

Teams: About eight teams.

Evaluations are Saturday, November 21 and Saturday, November 28, 2026, with basketball drills. Team games begin Saturday, December 5, 2026.

The fee is $100 per player. Fill in the form below first so Earl has your child's registration on file, then pay the $100 on the next screen. Nothing is charged before you submit this form.

Print the City of Philadelphia form

This one you cannot type into and sign on a screen. Its signature boxes only accept a digital certificate through Adobe Acrobat, and no parent has one. Print it on paper. It is a City document, so nobody is allowed to change it.

Sign the City form in pen and bring it in

Fill in every section by hand, including the medical, insurance, and doctor parts, and sign it. Bring the signed paper to McDevitt Recreation Center, 3531 Scotts Lane, on a Saturday and hand it to Earl or a coach.

Your child is not enrolled with the City until this signed paper is turned in.
